Supported Living Services is a program designed to help adult individuals live more independently by providing personalized support with daily activities, community engagement, and life skills in a home of their choice. We offer compassionate, tailored SLS support that empowers individuals to build confidence, routines, and meaningful connections.
Services include:
Daily living support (cooking, hygiene, housekeeping), Housing assistance and navigation, Budgeting and money management, Medical appointment coordination, 24/7 on-call support, Customized Independent Living Plans (ILP)
SafeHaven Team
Leadership & Team: Program Director: Allain Constantino
Team Includes: SLS Supervisor, Independent Living Coaches, Direct Support Professionals (DSPs), Case Managers, Nurse Consultant (as needed), Scheduling & Intake Coordinator
