ChoiceWorks empowers people with developmental disabilities, and their families, to design a life they love. We provide coaching, planning, and hands-on supports that put you in control of your services, budget, and team so you can reach meaningful goals at home, at work, and in the community.
What is ChoiceWorks?
ChoiceWorks is a Regional Center–vendored program offering two complementary service options for people choosing California’s Self-Determination Program (SDP): Service Code 099 (General Self-Directed Supports) and Service Code 024 (Purchase Reimbursement for Initial Person-Centered Planning and Pre-Enrollment Transition Supports). Together, these services help you learn the SDP process, create a person-centered plan, set your individual budget and spending plan, and get ready to enroll with the providers you choose—alongside an FMS (Financial Management Service), which is required in SDP. ChoiceWorks has a team of Independent Facilitators working with us and with clients under the Self Determination Program to assist with Person-Centered Planning.
At-a-Glance: Two services, two roles
099:
General Self-Directed Supports (G-SDS): We provide coaching and training for individual’s and their families after orientation and before enrollment so they can successfully transition into SDP, including step-by-step coaching to finish enrollment steps and tasks: understanding the waiver-approved service menu, drafting custom plans including the spending plan, choosing providers, and aligning their PCP to their IPP. Up to 40 hours authorized by the regional center, with additional hours available case-by-case when more intensive supports are needed.

024:
Purchase Reimbursement: Regional center reimbursement for the Initial Person-Centered Plan (PCP) and time-limited pre-enrollment transition supports from qualified non-vendored providers to get individuals ready for SDP. Including a written IPC plan that reflects individual’s strengths, preferences, culture, and vision for daily life and long-term goals, targeted pre-enrollment help (e.g., organizing documents, drafting your spending plan inputs, and coordinating next steps with the individual’s team), and more.
